I have witnessed and experienced this issue with myself, my wife and countless other buddies from UO.
There is no magic cure for the issue that happened to most UO playeras when the client went past 3.0.0c
I see that you are trying desperately, but in vain. It's all been done, tested many many many times with no real solution. The odd person might claim they have no issues and thats where the kanundrum is.
90% of us cannot use the 2d client, even if u lived next the server host and has a 20ms ping. The wierd horse pause thing after every few steps is not hardware related, it is not latency related, it is client related. As in the way your particular system utilizes resources to run it. There is no other explantion.
I experienced this phenonmenon when I se up UO on a linux computer beside my own. Surprise surprise the client lag
issue was gone.
It's a pity EA didn't address this issue waaaaaaay back when with a patch or a fix. It really seemed to go ignored for the loooongest time.
Most people cannot use the 2d client anymore for hunting and or PvP. If you are forced to use it due to not having the strong enough computer or ur trying to run the 3d client with your on bord video causes you massive display lag,(no surprise there) then you have to figure out why your system fails, where the other 10% succeed.
If you ever succeed please let me know, I gave up the quest long ago. I am happy with my ugly ugly non nostalgic 3d super fast client.(I guess :/)

I can I use 2D without many issues; the only time I have issues is when alot of action is going on IG for instance hunting in hyloth or some GM made level like salos lvl 4.
I have my good/bad days with 2D and I generally blame it on my ISP (because thats my scapegoat). I rarley ever "lag", my issues are freezies as mentioned above due to lots of IG action or something of the sort.
TBH:
If you, like myself cannot play 3D because of the same "spikes" but also because of the ugliness and the fact that when moving the cusor the display lags/becomes messed up, then you need to adapt your play style. I rarely pvp, when I do I use rites/when I was a knight close quarters sword and shield. Not everyone can be rambo. 3D users outrun me, but I don't mind because im not a pvp fanatic AND I love the way 2D looks. I Sold my liholites on the basis that my connection is not reliable and dragon breath can possibly make me lag.
All that is probably no help, just an example of what I go through. It's hard to explain my situation and tbh Im used to it now.

I use 2d 98% off the time it's great, but if you fight somebody who isen't a lootbag you need 3d (Talking about pvp) like Calamity, can't take her down in 2d becouse she can outrun me and heal etc.
I suggest you do following.
Use XP instead of Vista.
Open port 5003 on your router.
Patch to 5.0.9.1 exacly.

Yea, that's what I'm saying. I patched to 5.0.9 or whatever but it lets me play on 5.0.8 or whatever.

Here's the thing... My desktop has a 2.6GhZ processor, 3 gigs of ram, a 256mb video card, and a 7200RPM Hard drive. BUT, it runs 3D slower than old people screw, and it's using windows XP.
Laptop - My PvP machine - 1.4ghz processor, 128MB video card (dedicated), 1 gig of RAM, windows XP. Runs 3D seemlessly, and so far, the only person faster than me is Alexis.
The solution is SUPPOSEDLY the fact that my laptop is duocore processor, and the desktop is old school. That was what I was told, and apparently it's true.
